Patients aged over 75 years:
Patients aged over 75 are encouraged to book an annual health check appointment and those unable to attend the surgery are provided the same level of service on a home visit basis. A review of patients is completed to ensure an annual health check has been offered.
Patients not seen in over 3 years:
Patients not seen in over 3 years will still be able to access services at the practice provided they remain registered at the practice.
